Residential ~ Vocational ~ Structured Day ~ Outpatient
Beechwood NeuroRehab offers post-acute NeuroRehabilitation Residential and Outpatient services to persons 18 years and older who are medically stable, who need services and supports to maximize performance in everyday life or who need supported living services. Individuals served have acquired disabilities caused by either traumatic or non-traumatic injury to the brain, which is the result of trauma, surgery tumor, anoxia, stroke, or other neurological disorder. The persons served may demonstrate a wide range of cogitative impairments along with emotional or behavioral abnormalities, and drug or alcohol abuse disorders. The length of time in treatment is determined with the service team and the person’s individual needs and desires.
In addition to the above criteria, outpatients:
- Provide own transportation and lunch
- Must be able to safely administer their medications or do not need medication during the hours of program services 9:30 a.m. to 2:30 p.m.
When applying to Beechwood NeuroRehab, please send, if applicable, the most current applicant reports for pre-admission review and screening process:

- Individual service/support plan(s) such as Person-Centered Needs Assessment
- Neurological evaluation Report
- Psychiatric evaluation, if an applicant takes any medication for behavior control or has mental health diagnosis
- Physical exam report-specifying current diagnosis and medication list
- Progress reports or discharge plans regarding occupational, physical, speech, and cognitive Therapies
- Discharge summaries: most recent hospitalization and rehabilitation stays
Send referral information for residential, vocational, structured day, and outpatient services:

Include a cover letter with the following:
- Applicant’s full name and date of birth
- Give a reason for sending a referral to Beechwood NeuroRehab seeking residential or outpatient community-based supports and services.
- Please include referral source and/or funding source contact information for admissions staff follow up upon receipt of sent information.
